Office Policies

Once an appointment is scheduled, we will provide you with a patient information sheet and patient history form to complete.  You may fill out these forms prior to your visit to save time. To schedule an appointment please call one of our offices or make an online appointment request.

Cancellations:

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.

Emergencies:

Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week. Call (803) 779-3070 and one of our physicians will be contacted immediately

Billing & Payments:

cec-lex4picUpon completion of the examination, our patient advocate will ask for payment of your office visit, deductible, or co-payment of insurance benefits. Payment is expected at the appointment. We file most major insurance plans for medical diagnosis, but not for routine eye examinations. We will file all necessary insurance forms to facilitate timely payment of insurance benefits. We will schedule a follow-up appointment if indicated or set up a recall for your next routine appointment. You may then take your glasses prescription to the optical department for frame selection and glasses measurements.

Some insurance plans require referrals from the primary physician. Due to the insurance agreement, we cannot proceed with examinations without the appropriate referral. Please obtain the referral prior to your visit to insure that your visit proceeds smoothly.
